The field of vascular disease is rapidly eroding the artificial barriers that previously confined its practitioners to a rather narrow field of endeavor. To remain up to date, clinicians, teachers, and researchers in the field of vascular disease now must enlarge the scope of their knowledge to encompass a broad set of related topics.<br />

The Textbook of Angiology attempts to do this in an encyclopedic fashion, and for the most part it succeeds. The editors have endeavored to &#8220;ignore the classical disciplinary boundaries of the medical sciences&#8221; to form a &#8220;discipline that covers all diseases related to blood vessels, including coronary artery disease, hypertension, atherosclerosis, stroke, and peripheral vascular disease.&#8221;<br />
<strong></strong><br />
This comprehensive book covers vascular pathology, particularly atherosclerosis, and vascular pharmacology, including the medical management of coronary artery disease and lipid disorders.<br />
<strong></strong><br />
It also covers vascular radiology extensively, with discussions not only of modern diagnostic and therapeutic angiography but also of magnetic resonance imaging, nuclear-medicine techniques, and modern, sophisticated ultrasonography. There are comprehensive reviews of the biochemistry and therapeutic control of blood coagulation.<br />
<strong></strong><br />
The editors have organized the book into 15 sections and 103 chapters. Unavoidably, there is considerable redundancy and overlap in many of the topics. The first and by far the longest section of the book deals with physiologic principles in angiology.<br />
<strong></strong><br />
This section covers recent advances in vascular biology but is principally oriented toward coronary artery disease. The section also includes chapters on valvular heart disease and its medical and surgical treatment, as well as a chapter on minimally invasive direct coronary-artery bypass.<br />
<strong></strong><br />
The emphasis here is on the physiology of the coronary circulation, with little recognition of the concept that flow patterns and the clinical consequences of vascular obstruction in the coronary circulation may differ from those in noncoronary vascular beds.<br />
<strong></strong><br />
Subsequent sections describe the current status of peripheral vascular occlusive disease, mesenteric vascular disease, aortic and peripheral aneurysms, vascular problems in the arms, and cerebrovascular disease.<br />
<strong></strong><br />
The final sections deal with less frequently encountered vascular diseases, such as those with underlying inflammation, venous thrombotic disease, lymphedema, and unusual vascular malformations and hemangiomas.<br />
<strong></strong><br />
The print is excellent, and the 984 black-and-white illustrations are very clearly reproduced; most of them are original, rather than reproductions from other publications. Unfortunately, the reference lists vary in quality and style.<br />
<strong></strong><br />
Occasionally, quantity is substituted for quality, with some chapters citing more than 300 references. Furthermore, the use of such terms as &#8220;aspecific,&#8221; &#8220;monolateral,&#8221; &#8220;extremital,&#8221; and &#8220;exposition&#8221; (rather than &#8220;nonspecific,&#8221; &#8220;unilateral,&#8221; &#8220;extremity,&#8221; and &#8220;exposure&#8221;) is distracting. Many of the authors are from institutions outside North America, and their viewpoints are sometimes contrary to prevailing practices here.<br />
<strong></strong><br />
Examples include the statement that aortography is mandatory before repair of an abdominal aortic aneurysm and an endorsement of incidental appendectomy and cholecystectomy performed simultaneously with placement of a Dacron graft. Although the book has 1323 pages of text, a number of important vascular topics are omitted.<br />
<strong></strong><br />
Some common clinical problems and controversies encountered by vascular specialists on a daily basis, such as renovascular disease, fibromuscular disease, vascular infection, and carotid stenting, are not discussed.<br />
<strong></strong><br />
Vascular disease has evolved into an interdisciplinary specialty, as reflected by the broad scope of this book. This book will be particularly useful for the practitioner who seeks up-to-date information on the less frequently encountered aspects of vascular disease.<br />
<strong></strong><br />
It will thus be more useful as an occasional reference source on specific topics than as a broad overview of the traditional clinical aspects of peripheral vascular disease.<br />

The aim of the Textbook of Cardiovascular Prevention and Rehabilitation is to provide practical guidance for all cardiologists and other health personnel engaged in the management of cardiac patients. The book will follow the concept of &#8220;seamless care service&#8221;, i.e. a streamlined combination of preventive and rehabilitative methods in order both to add years to life and life to years. It will consist of eight sections.<br />
<strong></strong><br />
Commencing with an introductory section it will focus on guidelines, prevention, risk stratification and the evidence base of cardiac rehabilitation. In the following three sections different models for exercise testing and training, nutritional counselling and smoking cessation will be presented.<br />
<strong></strong><br />
The behavioral section contains health-related quality of life and psychological issues, patient education, compliance and adherence to the treatment program. In the social section the return to work, the role of relatives, heart foundations etc. will be discussed.<br />
<strong></strong><br />
Groups with special needs will be highlighted in the seventh section, which will address children with cardiac disease, elderly, women, heart failure and cardiac transplantation patients, patients with ICD or after valve surgery. The final section comprises evaluation options.<br />
<strong></strong><br />
quality assurance and organisational matters i.e. program design, staff training, safety, documentation and a health-economic cost review; even national program differences will be debated.<br />
<strong></strong><br />
Each section will contain a short summary and tables with practical guidance statements. Furthermore, all sections will be formatted as to follow the natural course of cardiovascular disease, starting off with focus on primary preventive care and finishing with the demands on rehabilitation of the patient with advanced disease, thus following the &#8220;seamless care&#8221; concept.<br />

The online version is very user-friendly and follows the same format as the textbook. One added bonus is the update feature where all the most recent advances are updated regularly and inserted into the relevant chapters. The CDROM contains a generous number of very well chosen, good quality slides which can be imported for use in presentations.<br />

I would strongly recommend this textbook as a worthy addition to the medical library of any primary or secondary care physician and the online version makes it a very practical reference tool for everyday use.<br />

The tracings are all graded for difficult; medical student, house officer, paramedic or nurse, and MRCP standard. electrocardiogram (ECG) can be a great leveller in medicine. It is common to see various combinations of medical students, nurses, junior doctors and specialists huddled around an ECG, debating what it shows. With his latest pocket sized text, John Hampton succeeds in further demystifying what in theory should be a relatively basic medical skill but in the real world sometimes proves to be quite the opposite.<br />

One hundred ECGs are shown, each with a brief clinical vignette and a management question. On the next page salient features of the ECG are described, followed by a clinical interpretation and brief discussion about the management of the condition described. The strengths of Professor Hampton s book are many. The combination of a brief history and clinically relevant question not only challenges the reader to give extra thought to the interpretation, but also makes the learning process far more enjoyable. The ECGs are randomly arranged rather than being grouped together according to the disease process or rhythm abnormality they represent, mirroring the manner in which medical personnel are confronted with ECGs in their day to day practice (or examination hall). Each ECG is also rated to indicate the degree of difficulty, allowing readers to make a rough self assessment of their level of competence.<br />

Although limited by the physical dimensions of the book, the clarity of the tracings is generally excellent, and the spectrum of ECGs shown (from normal variants through to a few clinical rarities) is a good representation of the problems encountered in current medical practice.<br />
